Procedures for Disclosure of Personal Information

For security reasons, we do not accept inquiries by telephone, e-mail, or fax. To make a request, fill out the appropriate request form and submit it by mail, together with the specified identification documents, to the Personal Information Inquiry Desk indicated below. Send by registered mail, delivery recorded mail, or any other method with a record of delivery. Please note that the cost of postage and other expenses required for the submission of necessary documents, etc. shall be borne by the requesting party.

  • Please note that we may contact you or your representative to proceed with the requested procedures.

  • We will not respond to requests made by the person in question or their representative in the following cases. Please note that if we decide against disclosure, etc., we will notify you to that effect and the reasons therefor.

    • If we are unable to confirm your identity, for example, owing to the information provided in the request form not matching the information we have on file

    • If we are unable to confirm the authority of representation for a request made by a representative

    • If we are unable to identify the personal information in our possession based on the information in the request form

    • In the case of other matters required by laws and regulations, etc.

  • Identity verification documents

    When the requester is the principal: Identification documents (valid or within 3 months of issuance, verifying name, address, and date of birth). Submit two of the following for identification documents that do not include a photo.

    • Copy of driver’s license (issued by each prefecture’s Public Safety Commission, excluding international driver’s licenses)

    • Copy of student ID card

    • Copy of Japanese passport

    • Copy of health insurance card

    • Certificate of residence showing current address

    • Copy of Disability Certificate, Rehabilitation Certificate, or Mental Disability Health and Welfare Certificate

    • Copy of alien registration card

    • Copy of passport

    When the requester is a representative: The following documents must be submitted in addition to the “Identification documents when the requester is the principal” above.

  • For legal representatives

    • Documents confirming that the applicant has the authority of legal representation (A copy of the family register issued within the last 3 months, or in the case of a person with parental authority, a copy of a valid insurance card with dependents [children] listed is also acceptable)

    • Documents confirming that the person has the authority of legal representation (Same as the documents for identification as set forth in “Identification documents when the requester is the principal” above)

  • For representatives by power of attorney

    • Our company’s prescribed power of attorney

    • Certificate of seal impression of the seal affixed to the power of attorney by the principal (valid within 3 months)

  • Charges

    Please note that a fee of 1,000 yen per application will be charged for each notification of the purpose of use and disclosure. Transfer the funds to the designated account, of which you will be informed after we receive the various application forms. (Transfer fees are the client’s responsibility.)

    * Please be aware that deletion or termination of use of personal information at your request may result in our inability to provide services in accordance with your requests.
